I understand that working with animals carries inherent risks, and that injury, including bites or scratches, may occur to me, my household members, or guests during participation in the Doggy Day Out program.*

I understand and acknowledge that as a volunteer for Charming Pet Rescue, I am not covered by workers’ compensation or any other insurance provided by the rescue. *

I will ensure the dog has a properly fitted collar with a Charming Pet Rescue identification tag and/or a properly fitted harness before leaving the rescue.*

I agree to keep the dog safely secured using an appropriate restraint or crate during transport to and from the rescue.*

I understand that Charming Pet Rescue will not cover or reimburse any expenses you choose to incur during the outing, including grooming, treats, or other discretionary purchases. *

I understand and agree that Charming Pet Rescue is not responsible for any property damage (including upholstery, carpets, or other belongings) or for illness or injury to other animals that may occur during the outing. *

If participating in an overnight Doggy Day Out, I understand that I am responsible for the dog’s care, safety, and supervision for the duration of the overnight stay, including providing appropriate shelter and access to fresh water.

I agree to hold harmless and release Charming Pet Rescue, including its past, present, and future officers, directors, employees, agents, and volunteers, from any and all liability for injury, illness, loss, or damage arising from my Doggy Day Out.*
